Folks: realise that the software is lying to you.

That is: when you post a reply, and get the message 'Software bug: reply failed' or whatever it is - DON'T BELIEVE IT! Your message *has* posted - and if you are fooled, and try to repost, you end up with duplicate posts. [If you feel the need, go to the forum and check: your post will be there.]

I as the high and might (and touchy) Site Admin, always dutifully delete multiple posts.

It's a bug in some of the code and I haven't bothered to clean it up. I know just enough phpbb to be dangerous.
