Here is the template I created for my army lists...
it is a combination of several different forms and has been tweaked as needed.

Very nice, muchly similar to one one I use - though I include scouting in the army composition section (helps auto calculation), and include a demployment map/scribble section at the bottom.

Though I cover the whole army on the one sheet - referring to generals as commands A, B, C etc then on the unit line have a column to nore which command they belong to (easy to sort).

I also added morale status (rout, shaken_ easily removed, and ran in wide format (no margins, wider than longer <is that right? >)
